  • Wellbore fluids can contain hydrocarbons only when the WCE system is in operation. Of the external leaks of wellbore fluids over the 15 month period shown, occurred while in operation and of those were in the BOP Stack subsystem. The external leak in operation was a leak at the telescoping joint that may have contained the abandonment brine being used at the time. The automatic failover switched to the redundant slip joint packer, and the leak stopped.
  • Events involving leakage of hydraulic control fluids are classified in the Control Fluid Leak, Internal Leak, and External Leak categories. Control fluid leaks of certain components can be considered as both internal and external leaks (i.e., the leak occurs both within and outside the component). To enable consistency in reporting, leaks from the following control system components are classified as control fluid leaks: ball valve, piloted operated check valve, ROV valve, electrically piloted directional control valve, hydraulically piloted directional control valve, quick dump valve, shuttle valve, regulator, and trigger valve.

  • To better understand and distinguish between normal wear and tear and unexpected failures, the "accelerated or early life failure" event outcome was added to the form on January 1, 2025, and is determined based on the time to failure and the type of component involved. Definitions are available in the User Guide for Reporting Well Control Equipment Failure. As data is collected, these definitions will be reviewed and may be revised.
  • Of the events classified as accelerated failures, failed within 3 months, were electronic () or elastomeric () failures within 1 year, involved metallic failures within 2 years, and involved failures in less than 5 years on components expected not to fail. Reporters may also identify "submitter defined" accelerated failures even though they fall outside of the SafeOCS definitions, though none have been submitted to date. All groups of accelerated failures are included in the chart.
